The Hypertension Management and Outreach (HMO) Program is a direct service and educational program available to adults with limited annual income or uninsured (no health insurance) with a primary diagnosis of high blood pressure (also known as hypertension).
The Hypertension Management and Outreach Program provides direct services including: blood pressure screening and assessment, referral to physicians, disease case management, and treatment. The program also provides counseling on lifestyle modifications including healthy eating, physical activity, smoking cessation, weight management, and medication adherence. Clinical and behavioral care guidelines are based on United States Preventive Health Services Task Force recommendations and other national standards.
